A BETTER WAY
We are an alternative to nursing home care that allows frail older adults to remain in their homes through a coordinated plan of care that includes medical, social and daily living support. But the best way to tell our story is through the life stories of those we help. People like John…
John, 90, has lived most of his life on Pittsburgh's North Side and attends our North Shore Adult Day Health Center Monday through Friday. Although John lives alone in a senior high rise apartment, he is a very social person and maintains many strong relationships with friends in the community.
John receives daily service from a LIFE Pittsburgh PCA that includes personal care, meal preparation, laundry and light housekeeping. John relies on LIFE Pittsburgh for grocery shopping and to help manage his banking and finances. At the Day Health Center, John participates in many activities, but also enjoys the opportunity to socialize and interact with others.
A DAY IN JOHN’S LIFE
8 am: PCA arrives at John's apartment to provide personal care, prepare breakfast, help with his portable oxygen tank and assist him to the van.
10:30 am: Non-denominational church service at LIFE Pittsburgh, led by the Chaplain. 11:30 am: Lunch prepared daily by LIFE Pittsburgh chef.
12:30 pm: PCA helps John with a shower in a private, handicapped accessible shower room.
1 pm - 3 pm: Participates in such activities as bingo, cards, trivia and exercises.
3 pm: PCA assists John from the van into his home.
5– 9pm: PCA arrives and prepares a meal for dinner, does some light housekeeping and laundry, helps John with his medications and prepares him for bed.
Because LIFE Pittsburgh is committed to serving only frail older adults who need long term care and are eligible for nursing home care, an outside opinion must confirm whether or not an individual qualifies for our services. The Allegheny County Area Agency on Aging will determine medical eligibility for the program after making an assessment of a potential enrollee’s needs. The Allegheny County Assistance Office will determine financial eligibility for the program, if applicable.
Qualification Requirements
To enroll in LIFE Pittsburgh, you must:
- Be at least 55 years old
- Live in one of the following 48 zip codes: 15006, 15007, 15015, 15017, 15031, 15044, 15046, 15056, 15064, 15071, 15082, 15086, 15090, 15091, 15101, 15106, 15108, 15116, 15126, 15127, 15136, 15142, 15143, 15201, 15202, 15203, 15204, 15205, 15209, 15210, 15211, 15212, 15213, 15214, 15216, 15219, 15220, 15222, 15223, 15224, 15225, 15226, 15228, 15229, 15233, 15237, 15241, 15243 (See Image Below)
- Be assessed as eligible for nursing facility level of care by the Allegheny Area Agency on Aging;
- Be financially eligible as determined by the Allegheny County Assistance Office or be able to privately pay; and,
- Be able to safely live in the community with services from LIFE Pittsburgh.
